Galleymaid head stator t bolt detached?!

I was investigating a no flush issue with our forward head. I pulled it apart to drop in a new stator and was surprised to find the t bolt was completely detached and laying in front of the stator. Has anyone ever had this happen before? I was sort of shocked. It went back together just fine and all is well now. I suppose it was just someone doing a poor stator service job?

Most likely though they tightened it down before they put it back together. Good help is so hard to find.
