I looked thru Sams Catalog and couldn't find the cover plate. Call Mike or a Steve at Sams and e mail this photo to them. I'm sure they know what it is and can either sell you a new one or tell you what they are.

Yes, the one on the left. I recently purchased the one on the right and it arrived in the same condition as the one pictured at Sam's.
