    You'd think someone would check before surfacing........nah, it's a big ocean, what's the chances?

    Too much time passed between their baffle clearing and rise to periscope depth, or someone utterly failed at their job, or there was a thermocline above the sub that prevented them hearing the surface ship as they rose. Either way... oops?
